IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
If you were enrolled in McLouth USD #342’s health plan, you may be entitled to a copy of Form 1095-C for the 2025 tax year. While this form is not required to file your personal income tax return, it provides information on your health coverage.
🔍 Where to Find Your 1095-C:
- Online Portal: You can view and download your 1095-C form directly from the Skyward employee portal. Simply log in and navigate to the ACA 1095 Forms on your employee screen.

- Request a Copy: If you prefer to receive a physical copy of your 1095-C form, you can submit a request by contacting the business manager. Your form will be mailed or emailed to you within 30 days of your request.
